Q: Is Cover
Florida a guaranteed issue policy?
A: Yes, Cover Florida
offers guaranteed issue policies to all individuals (age
19-64) and their dependents.
Q: Who is Cover
Florida available to?
A: Cover Florida is
available to individuals and groups 2-50 in size.
Q: Is Cover
Florida credible coverage?
A: Yes, the State of
Florida requires these plans to be credible coverage.
Q: What United
Healthcare network does Cover Florida utilize?
A: Enrollees have access
to United HealthCare's Options PPO Network.
Q: Is pregnancy
covered under Cover Florida plans?
A: Yes, pregnancy is
not treated as a pre-existing condition and would be
covered up to the policy limits.
Q: What do I need
to include in my submission for coverage?
A: Individuals
MUST include all of the following: individual
application, individual enrollment form and the first month's premium check. Groups
MUST INCLUDE all of the following: Employer application,
employer enrollment form and a
first month's premium check. Note: the premium check
must be mailed, but the forms can be faxed or e-mailed to
Benefit Advisors (do
not send submissions to your normal Florida Sales
office). If sending the forms by fax or e-mail, please
make a copy of the first page of the application and
include it with the mailed check so that we know what
application it belongs to.
Q: When will my
plan become effective?
A:
The deadline for the following month's enrollment is the
15th of the month prior to the effective date. If the
15th falls on a weekend, the deadline is 12:00 p.m. on
the Friday prior.
For example:
For an April 1 effective date - A scanned/faxed
copy of ALL completed submission paperwork (including a
copy of the check) must be received by Benefit Advisors
no later than noon on Monday, March 15, 2010. Your paperwork
will then be processed and your policy will take effect
on April 1, 2010. If your paperwork is not received by noon
on the 15th or the Friday prior, the effective date will
be pushed back another month.
Q: Is a 1-man
group the same as an individual?
A:
Yes, 1 life groups are the individual rates as
well as the rates for groups the size of one.
Groups must consist of at least TWO people (even if only
one is applying) in order to qualify for the group
rates.
