Insurance with Cover Florida.
Step 1:
There are currently two different Cover Florida plans
available through United HealthCare. Review both the
Standard and Standard Plus Group Benefit
Summaries and Group Rate Sheets, and choose the plan
that best fits your Employer Group coverage needs at
this time.

Step 2: Print both the
Employer Application and the Employee Enrollment form.
Each employee must fill out an employee enrollment. Fill each form
out completely and accurately.
