Individual Coverage

Please follow the steps below to apply for Individual Insurance with Cover Florida.

Step 1: There are currently two different Cover Florida plans available through United HealthCare. Review both the Standard and Standard Plus Benefit Summaries and Rate Sheets, and choose the plan that best fits your individual coverage needs at this time.

Step 2: Complete both the Individual Application and the Individual Enrollment Form. Make sure they are filled out completely and accurately. Contact one of our specialists if you have any questions regarding the application process.
